From ad: What you’ll be doing:
- Outreach Papa’s members by phone to validate needs identified, understand underlying barriers and take action to address them
- Communicate with members’ Care Teams (Care Coordinators, primary care physicians and other health care providers) to facilitate care
- Identify resources, including in-person Pal visit, that can assist members in overcoming care barriers like transportation, housing, food insecurity, and medication adherence.
- Follow-up with our members, to ensure their health needs are effectively met, and any obstacles they may face are promptly addressed
- Maintain a comprehensive knowledge of our partners’ health plan benefits including mental health, over-the-counter medicine allowances, and medical coverage, to ensure members understand and use all resources available to them
- Utilize internal applications to thoroughly document your interactions with members for reference
